Monitoring of maternal emotional adjustment, postpartum depression, and anxiety (referrals provided as needed)
Newborn screening drawn in the home
Guidance on breastfeeding (referrals to lactation consultants provided as needed)
Referrals for complications such as tongue tie
An additional 2 house call visits available during waking hours for other concerns beyond well visits; these complimentary house calls do not extend past 3 months of age.
1 – 3 years
Well visits at 12 months, 15, 18, 24, 30, and 36 months
Monitoring of growth and developmental milestones
Developmental screening and referrals to early childhood developmental services as needed
Autism screening at 18-30 months or earlier for at-risk children
Completion of school/daycare forms
Management of nutritional and developmental concerns
Guidance on safety and behavioral concerns
Telehealth, point-of-care testing, and office visits for urgent/sick visits (included in membership)
4 – 21 Years
Annual well-visits
Monitoring of growth and nutritional status
Management of chronic conditions such as ADHD (without comorbids), asthma, seasonal allergies, etc.
Mental health assessment and support (referrals as needed)
Sports physical exams and form completion
Telehealth, point-of-care testing, and office visits for urgent/sick visits (included in membership)
Completion of FMLA paperwork as needed
